A Care Coordinator/Wound Care/Education Nurse provides leadership and expertise in the development, implementation, and documentation of individualized service plans to meet the needs of Resident Veterans. This includes ensuring service plans are current and reflective of nursing interventions that address goals and needs of clients, completion of nursing assessments of clients seeking admission, readmission, or continued stay to determine criteria is met for needs being met at board and care level. In addition, will provide advanced nursing skills to provide wound/skin care, and assess, promote and educate nursing staff.

This position will travel between Hastings and Minneapolis Domiciliary Program locations.

The Minnesota Veterans Home in Hastings is a Domiciliary (DOM) which is an independent living community for Veterans. Qualifications

Minimum Qualifications Current Minnesota Registered Nurse License with no restrictions

Two years of experience as a Registered Nurse

Advanced knowledge of nursing sufficient to educate staff and residents

Knowledge of adult learning and training and development practices and methods, including needs assessment and instruction design

Strong interpersonal and communication skills to provide effective nursing education to groups of employees and individuals in classroom, one-on-one, and online training by using technology, equipment, devices and other facilitation tools

Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification

Ability to travel between Hastings and Minneapolis Veterans Homes

Wound Care certified or the ability to obtain certification within the probationary period
