JOB SUMMARY:

Face to Face Health and Counseling is a non-profit organization dedicated to meeting the needs of youth ages 11–24. We offer health care, counseling, and programs for homeless youth. The Drop-In Center (SafeZone) Case Management Supervisor provides direct case management services for youth at SafeZone. This person is also responsible for providing supervision and leadership for case management services at SafeZone. This position also trains case managers, ensures quality assurance protocols are in place, and guarantees case management practices align with funder requirements.

This position is 37.5 hours a week and requires availability on evenings.

PAY & BENEFITS: The pay rate for this position is between $51,000 and $57,500, based on qualifications. Responsibilities

SafeZone Case Management Supervision Supervise Case Managers at SafeZone (currently 6-7 staff); Ensure case management is provided in the context of the nine evidence-based principles for helping youth overcome homelessness (journey oriented, trauma informed, non-judgmental, harm reduction, trusting youth-adult relationships, strengths-based, positive youth development, holistic, collaboration); Develop and implement systems, protocols, and programs to manage the work of the case management team in order to ensure exceptional youth-centered care; Provide onboarding, training, supervision, and coordination of SafeZone Case Managers and interns; Provide case management services (short-term and intensive) to youth coming to SafeZone based on the nine guiding principles for helping youth overcome homelessness (journey oriented, trauma informed, non-judgmental, harm reduction, trusting youth-adult relationships, strengths-based, positive youth development, holistic, collaboration); Develop partnerships with state, county, and community agencies serving the same population in order to have access to resources for clients and facilitate referrals; Work with Manger of Homeless Youth Programs and the case management team to achieve goals and objectives of specific grants including tracking data necessary for grant reporting; Provide Case Consultation/Supervision to team on a regular basis; Maintain thorough knowledge of all grants and their requirements related to the work of case management services at SafeZone; Ensure timely and high-quality care for clients; Ensure compliance of all case management related work plans and reports, as required by funding sources; Provide feedback and input to grant proposals supporting case management services; and Work with case managers across the agency to develop and maintain agency quality assurance structures and protocols to ensure the provision of service with excellence. Step in to assist in day-to-day operations of the drop-in as needed

Case Management Work closely with SafeZone staff, including participation in work groups, case conferences, and other efforts to meet client needs; Co-facilitate workshops, groups and education classes for youth; and Provides all services guided by the evidence-based principles for helping youth overcome homelessness; and Perform other duties as assigned.

Qualifications

QUALIFICATIONS: Minimum Bachelor’s/equivalent degree in social work/human services OR equivalent experience required; Three or more years of case management experience, preferably with at-risk populations and/or youth; Solid assessment and case management skills; Excellent leadership and management skills, particularly with an ability to influence others without direct positional authority; Strong organizational skills and ability to manage multiple projects, deadlines, and competing priorities; High degree of problem-solving skill; Excellent verbal and written communication skills; Ability to work sensitively and effectively with diverse populations; Ability to work independently, exercising good judgment and involving others when necessary; Good crisis management skills and the ability to navigate stressful situations well; and Positive attitude and ability to work with and motivate a team.

PHYSICAL DEMANDS AND WORKING CONDITIONS: Ability to perform the duties outlined in the job description. Ability to perform duties and responsibilities typical of an office environment.
