Must have current RN license and must be eligible and have obtained a Minnesota Registered Nurse license and Public Health Nursing certificate by the time of employment. 

To provide a full range of public health nursing, health case management and community health services to individuals, families and the community; and to perform related duties as assigned.

Current vacancies are in the Clinical Services Division, Tuberculosis Control Program and Sexual Health Services.

Duties in the Tuberculosis Control Program involve managing all aspects of care for active TB cases throughout their treatment period.  Public Health Nurses (PHN) are responsible for ensuring that patients are fully treated for TB; this includes screening and managing for adherence and side effects, frequent communication and care planning with the providers in the Ramsey County TB Clinic, and ensuring directly observed therapy (DOT) is being completed. PHNs also make frequent home visits to provide a full range of holistic case management services including coordinating care with other healthcare entities, providing patient/family/community education, and assisting patients in getting their basic social service needs met. PHNs also assist TB epidemiologists in contact investigations. Experience working within diverse communities or with interpreters is desired. Nursing experience or interest in public health, infectious disease, homecare, and/or working with underserved communities is preferred.

 The Public Health Nurse in Sexual Health Services will provide independent assessment, planning, intervention and evaluations utilizing the nursing scope of practice for clients, families and communities in support of Sexual Health outreach, testing, syringe service and disease investigation. This position will require one evening shift per week until 7:00 pm and occasional work on weekends.

Vacancies may occur in several divisions at St. Paul - Ramsey County Public Health and Ramsey County Social Services, including the Family Health Division located at 1670 Beam Avenue, Maplewood, Screening and Case Management Division located at 160 E. Kellogg Blvd, St. Paul; and Clinical Services Division located at 555 Cedar St., St. Paul.

Examples of Work Performed

 

1. Perform independent health assessments of individuals, families and communities by evaluating their physical, mental, social and environmental health status; and provide nursing and community health intervention based on assessment of need and current community resources.
2. Make skilled nursing observations of client health status by assessing, monitoring and documenting such things as pregnancy, postpartum and infant/toddler status; developmental screening; depression screening; and evaluating the safety of home environments.
3. Share with clients, families, and communities methods of preventative positive health practices, such as health pregnancy; home and family safety; child growth and development; child spacing; and healthy parenting.
4. Provide case management for clients by developing care plans, consulting with other health care providers such as physicians and social workers, refer clients to other health care resources.
5. Provide orientation and training to Public Health Nurses, Health Educators, Dieticians, Home Health Aides, Health Education Program Assistants; and Public Health Licensed Practical Nurses by providing leadership in the area of case management, current medical and nursing technologies, current public health trends, and community resources.
6. Intervene in emergency situations by notifying proper authorities such as police, physicians, child protection agencies and personnel in medical facilities/emergency rooms, provide information to court officials in cases of child and adult protection.
7. Develop and provide health education and consultation services to community members, clients and family members, other professionals and Ramsey County employees on a one-to-one and group basis through home visits, clinics, health promotion and community outreach activities, respond to requests for information by phone or in person, coordinate and conduct health fairs, develop written and oral educational materials and curricula, and teach classes on topics related to public health issues, disease prevention and recognition, and safety and injury prevention, facilitate youth at-risk support groups on public health issues at local schools.
8. Serve on or chair internal and external work groups, task forces and advisory committees for the purpose of influencing and developing policy and procedures related to various community public health issues.
9. Assure privacy of client data according to federal and state regulations.
10. Maintain data related to activities and draft periodic reports to grantors as required.
11. Document cases, health promotion and community activities by preparing reports and necessary paperwork, develop and maintain required records and write case histories. Comply with child protection reporting requirements and collaborate with health care providers and community resources engaged with the family and schools.
12. Participate in team meetings and one-on-one meetings with supervisor.
13. Maintain data in a timely manner related to all activities as required.
