ORGANIZATION OVERVIEW: Face to Face Health and Counseling is a non-profit organization dedicated to advancing economic and health equity for youth ages 11–24. We do this work through a comprehensive approach that includes housing, mental health, medical, youth justice, education & employment, and community services. We take a strengths-based and trauma-informed approach to our work and center the needs of young people impacted by racism and other forms of oppression and marginalization. Founded in 1972, Face to Face serves more than 3,000 youth annually.

JOB SUMMARY:
This is an exciting new position for someone passionate about our mission who loves to work with youth to eliminate barriers to care and ensure patients receive our full scope of wraparound services! It includes case management, care coordinator, insurance assistance, appointment scheduling, and more.

This position is full-time at 37.5 hours a week. Availability required during all hours of operation: Monday, Tuesday, Wednesday and Thursday from 10:00 a.m. – 6:00 p.m.; Friday 10:00 a.m. – 4:00 p.m. Hours are subject to change depending on client needs and clinic hours.

LOCATION: Essential Onsite – This role requires the employee to be onsite.

PAY AND BENEFITS:
The pay rate for this position is between $24 and $25.50 an hour, based on qualifications. Face to Face offers benefits which include health, dental, short-term disability, long-term disability, life insurance, 403b account (3% employer match) as well as 23 days of PTO and 11 paid holidays.

Responsibilities

RESPONSIBILITIES: Clinic Care Coordination/Case Management • Assist in evaluating youth needs and barriers to care including food and housing insecurity, as well as a lack of access to basic needs • Coordinate care within Face to Face sites as well as with community partners in order to assure continuity and quality of care • Arrange transportation for clients to the clinic and other appointments as necessary • Act as the contact point for patient resources, advocacy, and crisis management for clinic patients • Assist with the case management and coordination of care for OB patients • Document services provided accurately and thoroughly • Provide assistance in coordinating medical care and community referrals beyond Face to Face to ensue patients who enter the clinic have access to all Face to Face services

Healthcare Access Coordination • Welcome the patients to the clinic with exceptional customer service • Assist with front desk operations including but not limited to: patient scheduling, transferring patient calls, scanning documents

Health Insurance Enrollment • Conduct one-to-one MNsure education, enrollment, and follow-up to uninsured youth receiving Face to Face services and uninsured community members seeking insurance • Assist with enrollment to MFPP • Maintain up-to-date knowledge of MNsure policies and Affordable Care Act (ACA) rules

Other Responsibilities • Model excellent internal and external customer service • Maintain patient confidentiality as required by agency’s policies and HIPAA • Assist other Face to Face departments with tasks and projects as assigned • Provides all services guided by the evidence-based principles for helping youth overcome homelessness; and • Perform other duties as assigned.

PHYSICAL DEMANDS AND WORKING CONDITIONS: • Ability to perform the duties outlined in the job description. • Able to perform duties and responsibilities typical of an office environment • Must be able to travel in all MN weather conditions • Face to Face is subject to the government mandate requiring employee COVID vaccination. All employees will be required to get vaccinated.

Qualifications

QUALIFICATIONS: • Associate or equivalent degree in social work or human services. Relevant work or lived experience considered in the place of a degree. • Minimum of one-year experience in case management preferred. • Demonstrated ability to work and communicate effectively with people of diverse ethnic, economic, racial and cultural backgrounds. • The ability to work independently, exercising good judgment in involving others when necessary. • Positive attitude and ability to work with a team. • Current MNsure Navigator certification; or the ability to be certified and pass a background check. • Good crisis management skills and the ability to navigate stressful situations. • Strong computer skills with a high level of attention to detail.
