Primary Function of the Position Hmong American Partnership seeks to hire a MNsure & SNAP Navigator who seeks a challenging, fast-paced work culture. The MNsure & SNAP Navigator is responsible for providing culturally and linguistically appropriate outreach and educational services to increase access to SNAP & MNsure as well as the delivery of program services primarily to the Southeast Asian / American and immigrant and refugee communities.
Position Responsibilities
- Educating and assisting Southeast Asian / American and immigrant and refugee community members of all ages about the purpose, use and benefits of assisted programs.
- Provide in-person and over-the-phone guidance for various assistant programs (MNsure, SNAP, Medical Assistance, MinnesotaCare, etc).
- Explain application process and program eligibility rules to the individuals.
- Assist individuals within the Southeast Asian / American community who have been traditionally hard to reach due to language barriers.
- Conduct home visits as needed to provide education and application assistance.
- Enter and update client information into data recording management system(s).
- Track and report outreach activities and outcome deliverables to supervisor in a timely manner.
- Track and capture case notations daily into Salesforce.
- Maintain up-to-date knowledge and information on public and private health insurance information and SNAP benefits.
- Help consumers communicate with program and county offices to resolve application related issues.
- Provide appropriate information and determine the appropriate referral within the organization's programs and/or referral partner organizations, agencies, and entities, as needed.
- Setting up and maintaining electronic case files; scanning and indexing documents and various clerical tasks; system photocopying, word processing, ordering supplies and forms, other clerical duties as assigned.
- Work with Leadership and Communications to develop educational and promotional material and content for outreach events, presentations and social media.
- Plan and execute outreach activities in a variety of settings such as HAP’s internal programs, schools, churches, community agencies, etc.
- Participate in all training requirements offered regarding assisted programs as well as department and agency trainings.
- Provide information in a way that is culturally and linguistically appropriate to clients.
- Other duties as needed to accomplish program and agency goals and outcomes.

Essential Functions

- While performing the duties of this job, the employee is regularly required to stand, sit, talk, hear, and use hands and fingers to operate a computer, phone, and keyboard.
- Must be able to remain in a stationary position for sustained periods of time in an indoor setting.
- Ability to tolerate moderate noise levels (i.e., shared open office space with computers, phones, printers, light traffic).
- Ability to sit, stand, stoop, kneel and reach overhead consistently.
- Must have the ability to move about the office to access files and office equipment.
- Frequent moving of boxes and files weighing up to 20 pounds.
- Requires occasional bending, kneeling, balancing, and reaching.
- Able to focus on data, analyze the information and draw conclusions.
- Frequent interactions with coworkers and participants requiring the ability to communicate effectively.
- Frequent communication in writing and via email requiring the ability to read and write in English fluently.
- Occasional preparation and analysis of figures.
Qualifications
Skills, Knowledge, and Abilities
- Bilingual is required (English and Hmong, Karen, Karenni, or Burmese).
- Excellent written and verbal communication skills.
- Experience and desire to work with people of diverse cultural, economic, educational backgrounds, and low-income families.
- Demonstrate sensitivity and empathy regarding human service issues.
- Ability to work independently and as a team member with minimal supervision in multiple locations.
- Ability to prioritize, manage multiple project responsibilities, and meet deadlines
- Knowledge of social determinants of health a plus
- The position requires flexible scheduling, including occasional weekends, late evening events, and travel within the 7-County metropolitan area surrounding Minneapolis and St. Paul.
- Must have reliable transportation, a valid driver's license, automobile insurance, and the willingness to use a personal vehicle for work-related travel.
- Ability to use various electronic communication tools, including Office 365 programs and databases.
Education and Experience
A minimum of a high school diploma: as Associates Degree in public health, social work, human services, or related field is preferred
At least two (2) years of relevant experience in community service or social work experience specifically around public health, healthcare and enrollment process preferred.
Knowledge and experience in the public assistance process are preferred.
Participant recruitment and outreach experience are strongly preferred.
