Job Description: Housing Stabilization Coordinators will assist individuals with disabilities in locating and maintaining adequate and affordable housing. The Housing Stabilization Coordinator is responsible for identifying barriers to housing and assisting clients through the process of relocating to community-based housing. Housing Stabilization Coordinators provide the needed support and advocacy for our clients along the way. In this role, you will begin by creating an individualized plan to identify barriers that have kept clients from finding long-term housing and try to help resolve those barriers with a whole picture approach to finding successful housing. Housing Stabilization Coordinators will continue working with clients after they've found housing. You will make a plan to help the client keep the home they've found. You will monitor the client's adherence to the plan throughout the year.
Responsibilities
Job Summary, Essential Duties and Responsibilities: Manage your own client caseload of 5-10 clients. Complete case notes daily, have the ability to write professionally. Identify and assist with resolving barriers to accessing housing. Conduct housing assessments and develop support plans that will help assist each of our clients receiving housing stabilization services (transitioning and sustaining) address their housing needs and housing barriers. Conduct housing searches, and build relationships with landlords, property managers and owners. Conduct in-home visits and remote work with those receiving services as well as within the community on a regular basis to address progress, and provide support. Liaise with external housing agencies, real estate professionals, and property managers to source appropriate housing opportunities. Develop innovative housing solutions for clients with unique needs, such as those requiring accessibility modifications or specialized medical equipment. Coordinate with contractors and vendors to ensure housing modifications are completed in accordance with medical requirements. Serve as an advocate for patients' housing needs within the healthcare system, advocating for suitable accommodations that contribute to their overall well-being. Address any challenges or obstacles that may arise in the housing process, working to find effective solutions. Provide education and guidance to healthcare professionals on the importance of housing stability for patient outcomes. Establish and maintain relationships with local housing agencies, nonprofits, and community resources to expand housing options for patients. Collaborating with the managing team by contributing your ideas and thoughts. We value your feedback as a team member! Building your knowledge of community, county and state resources for additional income assistance for our clients. Support the person in applying for all benefits that will help them afford their housing. Communicate effectively, positively and professionally with case managers, providers, family members and others, often from diverse backgrounds. Participate in community events and initiatives aimed at addressing housing disparities and promoting access to quality housing for vulnerable populations. Navigate Minnesota Healthcare programs and processes. Maintain files and program records.
Qualifications
Desired Experience: Case management: 1 year (Preferred) Housing Access Coordination: 2 years (Preferred)
Minimum Qualifications: Have a reliable vehicle with insurance and a driver's license to meet with clients if needed Must have a minimum of three months experience working in Housing Support. Must have a year of experience working with vulnerable adults / disabled individuals. Must reside in the Twin Cities or surrounding metro area to travel and meet with clients in the community. Proficiency working with Google suite & Microsoft Office. Professional references from previous employers. Empathy and sensitivity to the needs of diverse patient populations. Professional Working experience in speaking on behalf of the organization as well as the client Must be able to adapt easily, communicate with the team, follow company standards and protocols, work independently and productively manage their time. Problem-solving ability and a proactive approach to addressing housing-related challenges. Organizational skills to manage multiple cases and tasks simultaneously. Proficiency in relevant software applications for documentation and reporting. High School Diploma/GED certificate
Desired Qualifications: Bachelor’s degree in a social service-related field. Real Estate/Property management experience. Experience in providing direct social services. Experience in housing coordination, social work, healthcare navigation, or a related field. Knowledge of community and housing resources Experience assisting individuals with mental and/or physical disabilities and socioeconomic barriers.
