Face to Face Health and Counseling is a non-profit organization dedicated to advancing economic and health equity for youth ages 11–24. We do this work through a comprehensive approach that includes housing, mental health, medical, youth justice, education & employment, and community services. We take a strengths-based and trauma-informed approach to our work and center the needs of young people impacted by racism and other forms of oppression and marginalization. Founded in 1972, Face to Face serves more than 3,000 youth annually.
JOB SUMMARY:
The Coordinated Entry & Intake Specialist is responsible for conducting intakes at SafeZone, Face to Face’s day shelter for youth experiencing homelessness. This involves meeting with youth who are new to the space, collecting basic information, assessing their immediate needs, and connecting them to on-site and community resources. This role also serves as the primary Coordinated Entry Assessor for youth in Ramsey County. The Coordinated Entry System is an access point to dedicated housing for youth experiencing homelessness. This role will work with youth who are sleeping in a shelter, outside, in a place not meant for human habitation, or fleeing domestic violence to complete a Coordinated Entry Assessment. This assessment places youth on a prioritization list for housing programs based on their history and unique needs. This position is 37.5 hours a week.
Responsibilities
RESPONSIBILITIES: • Conduct intakes with youth coming into SafeZone, entering their information into our youth database (Apricot) quickly and accurately; • Connect new youth to services and supports available at Face to Face, particularly related to any immediate needs; • Conduct Coordinated Entry assessments with youth experiencing homelessness on an appointment and drop-in basis; • Enter assessments quickly and accurately into HMIS (statewide database); • Assist with other day-to-day tasks at SafeZone as needed; • Train new staff on conducting intakes and Coordinated Entry assessments; • Stay up-to-date on changes to CEYS and HMIS requirements and communicate changes to the broader team as needed; • Collaborate with county-wide Coordinated Entry partners to check on the status of youth on the priority list as requested; • Develop and maintain relationships with youth serving programs internally and externally; • Provide excellent customer service while providing intakes and CEYS assessments, conducting them from a harm reduction and trauma-informed perspective; • Provides all services guided by the evidence-based principles for helping youth overcome homelessness; • Engage with youth in a meaningful and respectful manner; • Respond to crises and de-escalate situations in a calm and professional manner; • Collaborate with other staff and maintain teamwork style; and, • Perform other duties as assigned.
Qualifications
QUALIFICATIONS: • High School degree or equivalent required; • B.A. in social services or equivalent education preferred; extensive experience in lieu of degree will be considered; • Minimum of 2 years work experience working with homeless or otherwise under resourced youth; • Ability to work independently, exercising good judgment in involving others when necessary; • Experience with a positive youth development and harm reduction approach; • Excellent interpersonal and communication skills; • High reliability and integrity; • Positive attitude and willingness to do what it takes to meet youth needs; • Good crisis management skills and the ability to navigate stressful situations well; • Knowledge of homelessness and its impact youth; • Demonstrated ability to work and communicate effectively with people of diverse ethnic, economic, racial and cultural backgrounds; • Good time management skills and ability to prioritize; and, • Strong attention to detail and ability to do data entry quickly and accurately.
