Position Summary: The primary duty of the Direct Support Professional is to provide support and advocacy in that clients can accomplish the desired outcomes of their Support Plan Addendum. This position requires the employee to train and monitor clients to work on facility-based contracts. In addition, this position will redirect, and assist in de-escalation tactics during challenging situations with clients. As well as engagement with clients, documentation requirements, and maintaining professional relations with coworkers and others associated with clients outside of the Company.

Responsibilities

Essential Responsibilities:

  1. Encourage clients to reach their personal and employment goals, including encouraging clients to work.
  2. Provide support, redirecting, and de-escalation to clients during challenging behavior situations.
  3. Assist with coping skills to help eliminate situations that may occur between clients and peers.
  4. Train and monitor clients to perform work-related contracts in accordance with the quality-standards of the facility-based customer.
  5. Set up and take down daily work contracts for client contract work.
  6. Encourage clients to engage in activities, outings, and community inclusion.
  7. Be engaged throughout the day with clients in activities or community outings.
  8. As needed, on Newtrax vehicles, ride with clients to and from Merrick.
  9. Complete client attendance and payroll, Pavillio, Data Sheets, communication books, provide input to incident reports and other required documentation for assigned clients.
  10. Transfer clients using appropriate body mechanics and assistive equipment such as wheelchairs, Posey belt, walkers, etc.
  11. Assist in meeting the transportation needs of clients including operating a motor vehicle, assisting in loading, and unloading of clients in a safe and reliable manner.
  12. Assist clients in personal cares such as, toileting, feeding, and preparing meals as needed.
  13. Assist in the preparation and upkeep of the program area to meet the needs of clients, employees, and the Company, such as sweeping, mopping, vacuuming, etc.
  14. In collaboration with the Program Plan Coordinator, provide input in client outcomes in accordance with Minnesota Statues Person-Centered Planning and Service Delivery.
  15. As needed, be cross trained and available for back-ups in other areas within the department.
  16. Follow any client or department specific safety measures.

Qualifications

Qualifications:

  1. High School Diploma
  2. Six to twelve months experience supporting individuals with Intellectual/Developmental Disabilities is preferred.
  3. Experience assisting clients with challenging behaviors is preferred.
  4. Ability to use electronic devices including databases and internet-based applications.
  5. Have a valid class ‘D’ driver’s license.
  6. Maintain a current DOT medical examiner certificate and medical waiver, is preferred.

  9. Ability to pass a criminal background check and drug/alcohol test following a conditional job offer as well as throughout the course of employment, required by driving policies.

Essential Physical Functions: The physical demands are representative of those that must be met by an employee to successfully perform the essential responsibilities of this position. The employee must be able to continually sit, stand, walk, and communicate throughout each day. Be able to stoop, bend, kneel, twist, run, reach, push, and pull, regularly move up to 10 pounds and occasionally move up to 50 pounds, including moving clients who are unable to support their own weight, push and pull wheelchairs used by clients weighing 100-200 pounds.
