JOB SUMMARY:

Face to Face Health and Counseling is a non-profit organization dedicated to meeting the needs of youth ages 11–24. We offer health care, counseling, case management, and programs for homeless youth. This position provides intensive children’s mental health case management services to children (up to 21) who meet criteria for the category of severely emotionally disturbed (SED) in Ramsey County and provides therapy for clients ages 11-25 years old. This position is full time at 37.5 hours a week. The position requires some evening availability.

PAY & BENEFITS:

The pay for this position is between $47,800 - $50,000 a year based on qualifications. Face to Face offers benefits which include health, dental, vision and life insurance, access to 403b and dependent care accounts, as well as generous PTO (23 days a year) and holidays (10).

Responsibilities

RESPONSIBILITIES for the Children’s Case Management role: Review diagnostic assessments of children with severe emotional disturbance to provide appropriate referral services for clients and families based on diagnosis and treatment needs Develop Individual Family Community Support Plans and Crisis Plans with child, family, professionals, and community, based on Diagnostic Assessment and functioning Assist the child and child’s family in obtaining needed services by referral to and coordination with other agencies to support goal progress for assigned clients Assure continuity of care and reassess the effectiveness of services over time Work closely with child’s school to support child in their individualized education plan Provide wrap-around support and care via coordination of meetings and contact with involved professionals such as health providers, child protection, school staff, and others who we have releases to speak with When clinically appropriate, assist in needs surrounding child’s placement and discharge from residential treatment, foster care and other services Prepare reports and make recommendations to court as required Maintain electronic and paper formats of case records, and necessary documentation in County SSIS system and Face to Face’s EHR system Collaborate with other staff and county team members Maintain professional working relationships with clients, other agencies, and contacts Participate in clinical supervision, group consultation, and staff meetings Establish healthy and appropriate physical and emotional boundaries with youth and their families Keep up to date with current changes in the field through the use of supervision, appropriate conferences, in-services and reading Manage and demonstrate the skills to respond to crisis situations by following protocols outlined in training

RESPONSIBILITIES for Outpatient Therapist: Provide culturally responsive mental health resources and therapy for youth ages 11-24, including diagnostic assessments, treatment planning, therapeutic case notes, collateral services, mental health referrals, and crisis management. Connect youth to long-term supports and programs appropriate to their mental health needs. Complete client documentation in compliance with DHS standards in a timely fashion. Build effective professional collaborations with community resources, providers, county and state workers, and partners to facilitate referrals and appropriate service provision. Provide services in a culturally responsive and linguistically appropriate manner. Build professional relationships with youth while helping them navigate the mental health system. Attend weekly Mental Health team meetings/consultations. Participate in individual supervision.

RESPONSIBILITIES for Both Roles: Collaborate with other staff and maintain team work style Comply with the agency’s written employment policies Model behavior consistent with, and supportive of, healthy adolescent development Establish and maintain healthy and appropriate physical and emotional boundaries with youth and their families Maintain professional licensure, if applicable, and uphold the ethical guidelines of your licensing board Provides all services guided by the evidence-based principles for helping youth overcome homelessness; Perform other duties as assigned

Qualifications

QUALIFICATIONS: Master’s degree in Social Work, Psychology, Counseling or Marriage and Family Therapy in a counseling discipline required Licensed as LMFT, LICSW or LPCC in state of MN preferred Minimum of 2 years’ experience required working with severely emotionally disturbed children and their parents Ability to work independently, use time effectively, work under stress and intervene appropriately in crisis situations Ability to work respectfully in diverse home settings Valid driver’s license, reliable vehicle, and current automobile insurance required Passion for working with Face to Face’s client population Demonstrated ability to work and communicate effectively with people of diverse ethnic, economic, racial and cultural backgrounds.

PHYSICAL DEMANDS AND WORKING CONDITIONS: Must be able to perform duties and responsibilities as outlined on the job description Able to perform duties and responsibilities typical of an office environment Must be able to travel in MN weather conditions to see clients in home settings May need to transport clients at times Face to Face is subject to the government mandate requiring employee COVID vaccination.
