Life Fountain’s Waiver Case Manager department is growing! This full-time role involves conducting assessments and developing support plans with individuals served while working in a bilingual setting. Ensure the support plan identifies the person's options and coordinate their choices of services and providers. There may be some minimal travel in this position.

This full-time role involves conducting assessments and developing support plans with individuals. In addition, ensure the support plan identifies the person's options and coordinate their best interest at heart. There has also been know to have minimal travel in this position.

 

 

Responsibilities

Below are a few more things to consider:

  • Would be conducting interview questions and taking notes to be entered in protected site for correspondences
  • Develop initial service plan with clients
  • Coordinate services between providers and clients
  • Schedule meetings with clients frequently or as needed to meet the needs of the client and demand of the service provided

Education

Preferred:

Experience with Rule 185, DD, AC/EW, CAC and/or CADI waiver preferred

Knowledge of MA, Medicare and MN Healthcare Programs strongly preferred

  • Background clearance
  • Effective communication skills with working proficiency in Word/Excel
  • Valid MN Driver's license, reliable vehicle you can use for work purposes, acceptable driving record and proof of auto insurance required
  • Spanish/English bilingual preferred
  • QDDP status

Qualifications

Requirements:

  • A bachelor's degree (at minimum) in social work, special education, psychology, nursing, human services or other fields related to the education or treatment of people with developmental disabilities or related conditions. Not done with your degree, no problem. We have Case Support positions available as well
  • One year of experience in education or treatment of people with developmental disabilities or related conditions, or a minimum of one course that specifically focuses on developmental disabilities.
  • Environment You will work as a part of a team that loves to help one another and meet weekly to make sure you are moving smoothly through your day to day and week to week.
