One of Ebenezer’s newest managed properties, The Sanctuary at West St. Paul offers 164 units of Assisted Living and Memory Care to a diverse population and acuity level of residents.  We are conveniently located near Interstate 494 and Highway 52 and public transportation.

The Director of Health Services (DHS) RN is responsible for providing leadership in all aspect of the provision of nursing services while ensuring regulatory compliance and service excellence.  While the Director of Health Services and Executive Director are ultimately responsible for ensuring that an RN is available to staff 24/7, Ebenezer provides on-call/afterhours nursing support to our assisted living sites using an efficient and effective triage system.  A trained triage nurse is available by phone to provide evaluation and direction for unexpected medical situations occurring at the community during evening and overnight hours.

This nursing position does support the entire Homecare staff within the Assisted Living/Memory Care Community. As a member of leadership, the position also serves in a manager on duty role as needed. Responsibilities

Oversees Memory Care and Assisted Living Resident Centered Care Ensures Clinical Compliance; develops nursing policies reflective of Ebenezer’s practice, oversees site compliance with Board of Nursing, Home Care Licensure, Hippa, OSHA, CLIA and Medicaid, and collects and analyzes health services data, trends and institutes actions for resolutions Manages nursing department and budget Provides leadership and general oversight of the nursing department Leads and/or performs coordination of resident care Supports a marketing plan aimed at full occupancy

Qualifications

Three to five years previous related experience Minimum Associates Degree, preferred BSN Must have current MN RN licensure and current CPR certification at start date
